Empire's Complex Care Program: Take the Call
Salesmen, scammers, and wrong numbers are behind most calls from unfamiliar sources. Your first thought might be to hang up immediately, or not to answer at all. But, if Empire Blue Cross Blue Shield is on the line, it’s in your best interest to pick up the phone. Empire’s our benefits partner and is calling to help you.
Empire has a Complex Care Program—that is completely free and confidential—for participants grappling with chronic conditions. The Complex Care Program can help if you have:
- Asthma
- Diabetes
- Cancer
- Heart Failure
- Chronic Obstructive Pulmonary Disorder
The Complex Care Program aims to personalize your care and give you peace of mind. Your program representative will take you step-by-step through the intimidating healthcare system, check in on you regularly and help you stick to your treatment plan. Every case manager with the program is a registered nurse ready to answer questions about your symptoms, insurance, prescriptions and more. You can access the program without even having to leave your home.
If you qualify for the program, you can expect a call from Empire. When they call, it will show up as a call from Empire Blue Cross on your caller ID. If you have not heard from Empire—and you think you qualify for the program—you can contact Empire yourself by calling (844)241-7089 or by visiting its website.
