How much is in my Sick Fund account?
This information can be obtained through the Member Portal. The Member Portal displays all sick fund transactions, including hours reported, contributions, payouts and the account balance. Please call 212-331-1800 if you need to assistance with accessing or activating your account.

Can you send me a list of the hours my employer reported?

Can I pick up my check?
All checks will be mailed to the mailing address that you have on file with Local 94. Checks will no longer be available for pick up in the office.
How do I submit my Sick Fund application?
Forms may be mailed to the Fund Office, emailed to sickfund@local94.com or faxed to 212-504-3292. Sick fund requests received prior to 11:30 am will be processed and mailed the same day. Sick fund requests received after 11:30 am will be processed and mailed the next business day.
How can I apply for Sick Fund benefits?
Your Sick Fund application form must be fully completed, and you must also have one of the following: (1) paystubs that show the day(s) for which you were not paid; or (2) your form must be signed by a supervisor. There are no exceptions.
It is my money. Why can’t I just take it?
The Sick Fund is a benefit covered by the terms of the collective bargaining agreement, and the rules of the Fund must be followed. Upon leaving covered employment, a member may file a claim for any unused benefit remaining in his/her account.
Why am I getting a W2 from the Health Fund?
The Sick Fund is part of the Health Fund. Members should receive a W2 for Sick Fund benefits received and/or Loss of Time Benefits from the Health Fund. These benefits must be claimed on your income tax return.
