Change of Address Information and Forms
It is the obligation of every member to be certain that the Union and Benefit Funds have their current address, phone number, cell number and email address. Please complete and send in the Contact Information form. The form must be fully completed and signed by the member.
Failure to keep the Union advised of your current address will result in a $50 fine in accordance with the by-laws.

You may contact Hach & Rose for assistance with on or off-the-job accidents; personal matters, asbestos claims, workers’ compensation; and social security disability issues.
Local 94 Financial Consultants
The Executive Board of Local 94 has elected to retain Stacey Braun Associates to provide Financial Counseling and advice to the membership of Local 94. (However, Stacey Braun Associates cannot provide specific advice on mutual fund recommendations in regards to the Annuity Fund. If you would like such advice, please click here.) You can receive one consultation per year in person or by phone and unlimited access to a financial help desk by emails or via their website. There is no out of pocket costs to members for these services.
Photo ID Cards
All members are required to obtain a Local 94 photo ID card upon enrollment or re-entry into the Union. Please visit the Union Hall during regular office hours to get your photo ID card.
FAQs
Why do I owe dues if I am on Dues Checkoff?
If you signed up for dues check-off, your monthly dues should be automatically withdrawn from your paycheck and sent by your employer to Local 94. However, sometimes an employer may not withhold your dues or your employer may withhold an incorrect amount. If this happens, you as the member are responsible for any unpaid dues, unless you can provide a copy of your paystub showing that your dues were withheld by your employer. If that happens, the Union will pursue the matter with your employer directly, and you are not responsible for paying any unpaid dues.
