How much is in my Sick Fund account?

This information can be obtained in person at the Fund Office with proper photo identification.

How do I get a Sick Fund Application?

Can you send me a list of the hours my employer reported?

Please contact Kimberly Antonaccio by email or phone at (212) 331-1826 and we will mail your records to your mailing address on file with Local 94.

Can my friend/spouse/co-worker pick up my check?

No. Only you, the member, can pick up your check in person. Checks can also be mailed to the mailing address that you have on file with Local 94.

When can I drop off my Sick Fund application?

You can drop your application off whenever the Union Office is open. However, Sick Fund checks are only issued in the mornings, from 8:30 AM to 11:30 AM.

How can I apply for Sick Fund benefits?

Your Sick Fund application form must be fully completed, and you must also have one of the following: (1) paystubs that show the day(s) for which you were not paid; or (2) your form must be signed by a supervisor. There are no exceptions.

It is my money. Why can’t I just take it?

The Sick Fund is a benefit covered by the terms of the collective bargaining agreement, and the rules of the Fund must be followed. Upon leaving covered employment, a member may file a claim for any unused benefit remaining in his/her account.

Why am I getting a W2 from the Health Fund?

The Sick Fund is part of the Health Fund. Members should receive a W2 for Sick Fund benefits received and/or Loss of Time Benefits from the Health Fund. These benefits must be claimed on your income tax return.
