Important Information Regarding COVID-19 Testing Requested By Employer

If your employer requires you to take a COVID-19 test, please do not use your IUOE Local 94 Health Fund insurance card (Anthem Blue Cross Blue Shield). Instead, request your employer's insurance information, so your employer can be billed. The payment for company required testing is your employer's responsibility, not the IUOE Local 94 Health Fund's. If your employer does not want to provide you with this information or you are having issues with this matter, please contact your Business Representative. See the attached letter from Local 94.

Why did I receive a bill for medical services?
While the Plan offers medical, prescription drug, dental and vision coverage, it does not pay 100% for all of these services. There are co-payments and coinsurance that you or your covered dependents may be responsible for paying.
