Over the past few weeks, the cases of COVID-19 have increased throughout the country, including the metropolitan area. It is understandable that the public is nervous. Be assured, our Union is following the CDC’s lead, along with advice from the New York Department of Health to keep members and their families safe.
Currently, our administrative offices remain open, but we are requesting that members limit their visits to our office to services that are essential and cannot be completed via e-mail or by telephone. We suggest visits to the office to see the administrative staff or business agents be reserved for emergencies. Please see our Member Reference Guide for more information.
Please continue to e-mail all sick fund claim forms to sickfund@local94.com. This will be the most efficient way to receive your Sick Fund claim forms. Forms can still be mailed, but the claims processing will take longer. All Sick Fund checks will be mailed to participants. Forms received prior to 11:30am will be processed and mailed that day. Forms received after 11:30am will be processed and mailed the next business day. You can also fax us at (212) 504-3292.

FAQs
How do I add my newborn child to my plan?
You must provide the Fund Office with a copy of the child’s birth certificate within 90 days of birth to enroll your child with the applicable date (child’s birthdate). If you fail to do so, within the applicable 90-day period, dependent coverage will not be available under the Plan for your new dependent child until the first of the month following the date in which you provide the Fund Office with the required documentation or any other verifying information requested. A Coordination of Benefits Form (COB) must be completed if your child has other coverage. You should also visit the Life Events page on this site to see what you need to do for your other benefits.
